CASE N°33 – Mediastinal TBFNA, spindle cell carcinoma

Clinical History:

Age: 51

Sex: Male

bronchial biopsy in July 2013 with a diagnosis of squamous cell carcinoma
pulmonary lobectomy in September 2013 with a diagnosis of moderately differentiated squamous cell carcinoma
after almost a year PET positive mediastinal lymph nodes

Cytological Report:

Positive for epithelial malignant tumor cells in part with keratinized cytoplasm

Histological Report:

Localization of squamous cell carcinoma

Discussions:

A,B) at low magnification you can see clusters of epithelial cells

C-E) here you can see clusters of epithelial cells with orangiophilic cytoplasm and polymorphic nuclei

F,G) at higher magnification you can more clearly observe the atypia of chromatin and conclude that these are malignant tumor cells from spindle cell carcinoma

H-M) biopsy confirm our cytological suspect
